29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE):  On or about July 22, 2016, and in times prior thereto and thereafter, Kitchen workers were exposed to such hazards as hot ovens, flying objects, slips and falls.  A PPE survey was not conducted to determine the appropriate protection.

29 CFR 1910.138(a): The employer did not select and require employee(s) to use appropriate hand protection when employees' hands were exposed to hazards such as those from skin absorption of harmful substances; severe cuts or lacerations; severe abrasion; punctures; chemical burns; thermal burns; and harmful temperature extremes.  On or about July 22, 2016, and in times prior thereto and thereafter, Cooks were not provided with cut resistant gloves, gloves for touching hot surfaces and cut resistant aprons. This condition exposed them to cuts and burn injuries, located in the kitchen area.

29 CFR 1910.138(b): The employer did not base the selection of the appropriate hand protection on an evaluation of the performance characteristics of the hand protection relative to the task(s) to be performed, conditions present, duration of use, and the hazards and potential hazards identified:  On or about July 22, 2016, and in times prior thereto and thereafter, cooks were wearing gloves around the rotating parts of a meat grinder, exposing them to the hazard of their hands and arms being pulled into the grinder.  The grinder was located in the kitchen area.

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  On or about July 22, 2016, and in times prior thereto and thereafter, lockout/tagout procedures were not established for cleaning and maintaining the Cabela's meat grinder, exposing employees to the hazards of being caught in equipment during cleaning.
