29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: (a) North Roofing, LLC at 2705 46th Street, Loveland, CO 80538: On or before July 27th, 2016, the employer did not ensure that the employees were wearing safety glasses when using or working near pneumatic nail guns and circular saws. Condition exposed employees to hazards of projectiles to the eyes.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501(b): (a) North Roofing, LLC at 2705 46th Street, Loveland, CO 80538: On and before July 27th, 2016, the employer did not ensure that each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was protected from falling. Employees were shingling roofs with up to a 6 on 12 pitch roof, and were working without fall protection. This condition exposed the employees to a fall hazard of up to approximately 25 feet.
