29 CFR 1926.150(c)(1)(vi): A fire extinguisher, rated not less than 10B, was not provided within 50 feet of wherever more than 5 gallons of flammable or combustible liquids or 5 pounds of flammable gas were being used on the jobsite:     (a)  Jobsite:  On or about July 25, 2016, the employer did not have any fire extinguishers on the site.   There were 12 full, 5 gallon cans of flammable paint at the site.

29 CFR 1926.453(b)(2)(iv): Employee(s) working in an aerial lift were not standing firmly on the floor of the basket:     (a)  Jobsite:  On or about July 25, 2016, an employee was standing on the mid-rail of an aerial lift while he was painting the side of a tank.  The employee was not tied off and the fall hazard was about 35 feet to the ground.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system:     (a) Jobsite:  On or about July 25, 2016, an employee was working from the basket of an JLG 600S aerial lift and he was not wearing a safety harness nor was he tied off.  The fall hazard to the employee was about 35 feet to the ground.
