29 CFR 1910.134(d)(1)(ii): The employer did not select and use a NIOSH-certified respirator in compliance with the conditions of its certification:  a)    Colonna's Shipyard, Vessel Andrew J. Barber: On or about 07/19//2016, subcontractor employees engaged in abrasive blasting wore MSA supplied air respirators with hoods that were not used in compliance with the conditions of their NIOSH certifications. The red 3/8 inch hose connected to the respirator tube was not an approved MSA supplied component.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.134(i)(5)(iv): The employer did not ensure that the person authorized to perform the necessary sorbent bed and filter changes filled out a tag indicating by the signature and date that maintenance changes were performed:  a)    Colonna's Shipyard, Vessel Andrew J. Barber: On or about 07/19/2016, the RADEX airline filter provided by the contractor and used by the subcontractor employees, a tag containing the signature and the date of the last maintenance change was secured to the compressor.   Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.134(j): The employer did not ensure all respirator filters, cartridges and canisters used in the workplace were labeled and color coded with the NIOSH approval label and that the label was not removed and was legible:  a)    Colonna's Shipyard, Andrew J. Barberi : On or about 07/19//2016, warning labels for the two RPB RADEX airline filters were missing as a result contributed in the improper use of the filters.   Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1915.12(d)(1): The employer did not ensure that each employee entering a confined or enclosed space or other areas with dangerous atmospheres was trained to perform all required duties safely.  a)    Andrew J. Barberi, Aft Peak Tank, On or about 07/18/2016, the employer did not train new employees in hazards associated with confined spaces prior to entry. Employees entered the tank to pressure wash and conduct quality control inspections.   Abatement certification required within 10 days after abatement date.
