29 CFR 1926.760(d)(2): Fall arrest system components used in fall restraint systems did not conform to the criteria in 1926.502. On or about 7/20/16, at the Seaton Hall Renovation Project, Kansas State University an Iron Worker was performing bolt connecting work using a personal fall arrest system that was not configured to prevent free fall at less than 6 feet and prevent contacting lower level hazards. Employee was exposed to struck against and impalement hazards.

29 CFR 1926.761(c)(2)(ii): The employer did not ensure that each connector had been provided training in the establishment, access, proper connecting techniques and work practices required by section 1926.756(c) and section 1926.760(b). On or about 7/20/16, at the Seaton Hall Renovation Project, Kansas State University an Iron Worker was not trained to recognize varying fall hazard distances and to use proper length fall protection devices to minimize struck against hazards. Employee was exposed to struck against and impalement hazards.
