29 CFR 1910.147(c)(7)(i)(A): Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control. a) On June 16, 2016 employees who clean and apply tape on the Deco Booth was not trained as an authorized lockout employee in that during the cleaning and applying tape employees were exposed to struck by hazards from the frame/mast coming down upon their arms and hands.

29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: a) Employee who perform servicing and/or cleaning on or around the Deco Line were not required to : (1) Physically locate and operate the electrical and air disconnect in such a manner as to isolate the machine or equipment in that on June 16, 2016 while operating the Deco Booth the upper mast/frame came down upon the operators arm, pinning and holding the operators arm inside of the machine exposing them to crushing and/or struck by hazards. (2) Affix a lockout or tagout devices to the electrical and air disconnect
