29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.  a) Located along various peak areas of the rear roof deck church structure with a pitch of 14 in 12 there was an employee observed performing roofing work associated activities without conventional fall protection exposed to fall potentials in excess of 11 feet.

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, shall be rigged such that an employee can neither free fall more than 6 feet (1.8 m), nor contact any lower level;  a)Located along the rear roof deck eave and gutter of the central church structure with a pitch of 14 in 12  there was an employee observed performing roofing drip edge work associated installation activities utilizing a vertical lifeline and shock absorbing lanyard fall arrest system attached to the employees harness. The employee was exposed to a swing fall hazard at the unprotected rake side or edge area of the roof deck and a 12 foot swing fall strike against the projecting side lower level of the structure.

29 CFR 1926.1053(b)(8): Ladders placed in any location where they can be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, shall be secured to prevent accidental displacement, or a barricade shall be used to keep the activities or traffic away from the ladder.  a) Located along the rear area of the church structure roofing employees were observed gaining access to the roof area of the structure by utilizing an extension ladder which was placed directly in the swing path and exit/entrance rout of two exterior doorways without the extension ladder being secured from displacement or barricaded exposing employees utilizing the ladder to fall potentials in excess of 11 feet.
