29 CFR 1910.178(k)(1): The brakes of highway truck(s) were not set and wheel chocks placed under the rear wheels to prevent truck(s) from rolling: Shipping and Receiving Yard / On July 24, 2016 the employer exposed employees to a crush by hazard in that the waste trailer did not have wheel chocks in place when loading with a forklift.

29 CFR 1910.178(l)(1)(i): The employer shall ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l). On July 24, 2016 the employer exposed employees to a crush by hazard in that safe operating procedures were not being used while moving a waste trailer from the dock with a yard dog.
