29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed.  Electronics Maintenance Unit; on or about 28 July 2016 and times prior thereto, annual periodic inspections of the employer's energy control procedure (Lockout/Tagout) was not conducted. Last review was conducted on 08 September 2014. This condition exposed to struck-by, caught-between and electrical shock hazards.

29 CFR 1910.212(a)(5): Fan blade guard(s) were not provided where the periphery of the blades was less than seven feet from the floor or working level.  Maintenance shop, upper storage area; on or about 28 July 2016 and times prior; a ceiling mounted Modine unit heater, periphery fan blades were not guarded with protective barrier one-half inch or less. The measured distance from the floor to blade was 52 inches. This condition exposed employees to a caught in hazard.

29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed.  Back-up Generator Building and Computer Room; on or about 28 July 2016 and times prior thereto; two instances were noted where energized electrical openings were not effectively closed:             a. Back-up Generator room, back wall; an energized electrical wall outlet was missing a knockout plug on the left hand side.       b. Computer room, an electrical panel labeled PPC2; was missing a 1 inch by 2 inch blank cover for breaker 20.   This condition exposed employees to electrical shock and electrical burn hazards.

Anchorage Forecast Office; on or about 28 July 2016 and times prior thereto; the employer did not provide the OSHA Form 300A  Summary of Work-related Injuries and Illness and OSHA Form 300 Log of Work-Related Injury and Illness Logs for 2014 within four business hours. This condition leads to inaccurate reporting of injuries and illnesses.  The National Oceanic and Atmospheric Administration was previously cited for a violation of this occupational safety and health standard 29 CFR 1904.40(a), which was contained in OSHA inspection number 943640, citation number 2, item number 1c and was affirmed as a final order on 11 November 2013, with respect to a workplace located at the Anchorage Forecast Office Maintenance shop.

29 CFR 1910.242(b): Compressed air used for cleaning purposes was not reduced to less than 30 p.s.i.  Facilities Maintenance Shop; on or about 28 July 2016 and time prior thereto, an air nozzle used for cleaning purposes exceeded 30 p.s.i. Air nozzle measured approximately 85 psi. Air nozzle was not equipped with deflector to reduce the pressure of air flow. This condition exposed employees to foreign objects being embedded to the body and eyes.   The National Oceanic and Atmospheric Administration was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.242(b), which was contained in OSHA inspection number 943631, citation number 1, item number 3 and was affirmed as a final order on 11 November 2013, with respect to a workplace located at the Anchorage Forecast Office Maintenance shop.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket).  Maintenance shop; on or about 28 July 2016 and times prior thereto, two instances were noted where portable electrical plug equipment was missing the ground pin.      a. A Rigid Model 300 Pipe Threader,  serial number 772664 was missing a ground pin. This condition exposed employees to an electrical shock hazard.      b. An blue and yellow striped electrical extension cord was missing the ground pin.    This condition exposed employees to an electrical shock hazard.   The National Oceanic and Atmospheric Administration was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.334(a)(2)(i), which was contained in OSHA inspection number 943631, citation number 1, item number 6 and was affirmed as a final order on 11 November 2013, with respect to a workplace located at the Anchorage Forecast Office Maintenance shop.
