29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:  On or about at 7/28/16 located at 6905 Branford Dr., Cumming, GA: Employees were roofing a   house that was under construction with an 6/12 & 10/12 pitch. Employees were installing roofing shingles on the roof and the excess shingles were falling back down to the ground, employees were observed without hard hats exposing them to a struck by hazards.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.  On or about at 7/28/16 located at 6905 Branford Dr., Cumming, GA: Employees were exposed to eye injury, where employees were using a pneumatic nail gun without safety glasses while installing the roofing shingles.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about at 7/28/16 located at 6905 Branford Dr., Cumming, GA: Employees were installing roofing shingles while standing on the edges of a house that was under construction working at the edge, the employer did not ensure that fall protection was used by the employees while exposed to fall hazards of approximately up to 22  feet and with a pitch of 6/12 & 10/12.
