29 CFR 1926.1408(a)(2): The employer did not determine if any part of the equipment, load line or load (including rigging and lifting accessories), if operated up to the equipment's maximum working radius in the work zone, could get closer than 20 feet to a power line: On or about July 21, 2016 at the jobsite located at 1101 Indiana St., Lawrence, KS, an employee conducting rigging activities was exposed to electric shock hazards in that the employer did not determine the safe radius of the work zone, allowing parts of the equipment to get closer than 20 feet to the power line.

The employer did not train each employee specified in � 1926.1408(g) and � 1926.1410(m) in the topics listed in � 1926.1408(g): On or about July 21, 2016, at the jobsite located at 1101 Indiana St., Lawrence, KS an employee conducting rigging activities was exposed to electric shock hazards in that the employer did not provide employees with training to recognize safe work distances from overhead power lines to prevent electric shock hazards.
