29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    a. On or about July 26, 2016, front of home, the 20-foot metal extension ladder was not extended 3 feet above the landing.  This exposed the employees to fall hazards.

On or about July 26, 2016, the employee using a nail gun to install roofing shingles was not wearing safety glasses and being exposed to eye injuries.     Collis Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 472520, citation number 01, item number 001 and was affirmed as a final order on August 29, 2012, with respect to a workplace located at 2120 White Jasmine Court, Apopka, FL 32703.

The employer fails to assure effective protection against falls as required by the standard, as demonstrated on July 26, 2016, when employees were working on a residential roof at 202 Wellington Woods Ave, Deland, Florida 32750 without fall protection and were exposed to a 12 feet fall hazard.     Collis Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 721522, citation number 02, item number 001 and was affirmed as a final order on February 3, 2014, with respect to a workplace located at  204 38th Avenue South, Jacksonville Beach, FL 32250.    Collis Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 769902, citation number 01, item number 001 and was affirmed as a final order on February 3, 2014, with respect to a workplace located at 4751 Silver King Way, Palm Harbor, FL 34684.
