29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) Owens Illinois, Inc. on or about July 26, 2016, at the site located at 1 Michael Owens Way (plaza #1) Perrysburg, OH: The employer did not ensure that machine specific energy control procedures were developed and utilized to control potentially hazardous energy sources while an employee performed service and maintenance work on the roof top mounted Air Handler Unit #2. The employee performing this activity in a condition in which electric and mechanical energy was not isolated, was exposed to incoming nip points at the air handlers belt and pulley drives.

29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s): a) Owens Illinois, Inc. on or about July 26, 2016, at the site located at 1 Michael Owens Way (plaza #1) Perrysburg, OH: The employer failed to ensure that hazardous energy sources such as, electrical and mechanical potential were physically isolated before employees performed service and maintenance work at or near belt and pulley drive(s) contained in the roof mounted Air Handler Unit #2. An employee performing this activity in an uncontrolled condition was exposed to incoming nip points at the units drive belt and pulley, in that energy isolation devices required under 29 CRF 1910.147(d)(3) were not utilized before the employee began the aforementioned work task.

29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that employees acquired the knowledge and skills required for the safe application, usage and removal of energy control devices: a) Owens Illinois, Inc. on or about July 26, 2016, at the site located at 1 Michael Owens Way (plaza #1) Perrysburg, OH: The employer failed to ensure that maintenance employees tasked with service and maintenance activities at or near energized parts of the roof mounted air handler units were adequately trained in the skills required for the safe application, usage, methods, and means of isolating and controlling hazardous energy sources. Employees tasked with service work on the aforementioned machine were not specifically trained to lockout the machine to protect against amputation injuries prior to performing work at or near nip points and moving parts.
