29 CFR 1910.134(h)(3)(ii)(A): The employer did not ensure that respirator inspections included a check of respirator function, tightness of connections, and the condition of the various parts including, but not limited to, the facepiece, head straps, valves, connecting tube, and cartridges, canisters or filters: a) Manufacturing Area - On or about 7/26/16, the employer did not ensure that respirator inspections of MSA half-mask air-purifying respirator(s) (employer required use) did not include valves as the respirator was missing the inhalation and exhalation valves were missing. ABATEMENT CERTIFICATION REQUIRED

29 CFR 1910.176(b): Bags, containers, bundles., etc, stored in tiers were not stacked, blocked, interlocked, or limited in height so that they were stable and secured against sliding or collapsing: a) Throughout facility - On or about 7/26/16, super-sacks, pallets of bagged materials, and pallets of 55-gallon drums were not stacked in a stable manner exposing employees to being struck by these materials when the sacks or pallets slid or collapsed.
