A. Pyramid Roofing and Remodeling LLC worksite located at 12092 Dowling Rd., Bowling Green, Ohio: On or about July 26, 2016, the employer did not ensure employees were provided with proper eye protection while operating pneumatic nail guns. Two employees were exposed to potential flying object hazards while operating the pneumatic fasteners.

29 CFR 1926.501(b)(1) Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. A. Pyramid Roofing and Remodeling LLC worksite located at 12092 Dowling Rd., Bowling Green, Ohio: On or about July 26, 2016, the employer did not ensure employees were protected from a fall distance of approximately 13.00 feet. The exposed employees were performing weatherproofing on the 8/12 pitch barn roof without any form of fall protection in place.
