29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition: (a) Main Dock, Compactor area: On and about 07/29/2016 employees were exposed to unsanitary conditions where syringes, broken glasses, biological waste, and other trash were strewn on the floor and on the ground. (b) Chutes areas: On and about 07/29/2016 employees were exposed to unsanitary conditions where there were blood stains and other wastes on the floors.

29 CFR 1910.138(a): The employer did not select and require employee(s) to use appropriate hand protection when employees' hands were exposed to hazards such as those from skin absorption of harmful substances; severe cuts or lacerations; severe abrasion: Chutes and Compactor areas: On and about 07/29/2016 employees who were exposed to puncture hazards from sharp objects such as, but not limited to, needle sticks introduced into trash containers which were processed downstream, were not provided puncture resistant hand protection. There had been incidents of needle stick injuries in the past as reflected on the OSHA 300 logs.
