29 CFR 1910.23(a)(8): Every floor hole into which persons can accidentally walk were not guarded:      (a)  On or about June 15th, 2016, and at certain time before, employees working in the security and engineering departments at the employer's 1213 Wyandotte, Kansas City, Missouri, facility were exposed to serious trip and fall hazards due to a number of floor hole covers on multiple floors not being secured in place, and two larger floor hole covers on the 8th and 9th floors were noted to be rotting and incapable or supporting a worker's weight.  Unguarded floor holes were also noted to exist on multiple floors where the central utility lines penetrated each floor.

29 CFR 1910.23(b)(1): Every wall opening from which there was a drop of more than 4 feet was not guarded as required by standard:   (a) On or about June 15th, 2016, and at certain times before, employees working in the security and engineering departments at the employer's 1213 Wyandotte, Kansas City, Missouri, facility were exposed to falls of greater than 4 feet due to a number of unguarded wall openings on multiple floors where the central utility lines penetrated each floor.

29 CFR 1910.23(c)(2): Runways four feet or more above floor or ground level were not guarded by standard railings (or the equivalent):      (a)  On or about June 15th, 2016, and at certain times before, employees working in the security and engineering departments at the employer's 1213 Wyandotte, Kansas City, Missouri, facility were exposed to serious fall hazards while walking on or working from the multiple catwalks above the main ballroom due to a number completely unguarded sides and edges existing at various locations throughout the catwalk array.  In some locations where this hazard existed the catwalks appeared to be less than 4 feet above the next lower level, but that "lower level" was actually a suspended acoustical tile ceiling that would allow an employee to fall through to the lower floor level.

29 CFR 1910.37(b)(6):  Each exit sign was not illuminated to a surface value of at least five foot-candles (54 lux) by a reliable light source:      (a)  On or about June 15, 2016, and at certain times before, employees working at night in the security and engineering departments at the employer's 1213 Wyandotte, Kansas City, Missouri, facility were exposed to possible fire, smoke, and entrapment hazards due to a number of exit signs not being illuminated or otherwise visible in a dark environment.
