29 CFR 1926.403(b)(1)(vii): 1926.403(b)(1)(vii):   Safety of equipment was not determined on the basis of the following considerations: Other factors which contribute to the practical safeguarding of employees using or likely to come in contact with the equipment:   (a)  Jobsite, Basement Electrical Room:  On or about July 26, 2016, the employer selected 3/4 inch long self tapping, 5/16's bolts to secure the trough cover.   This trough contained energized conductors.  This condition created the possibility of the tip of a self tapping bolt to come in contact with the conductors inside its insulation.
