29 CFR 1910.132(d)(1)(i): The employer did not select and have each affected employee use, the types of personal protective equipment that would protect the affected employee(s) from the hazards identified in the hazard assessment:    (a)    A hazard assessment had not been performed for the chemical blending task involving addition of corrosive materials.   Incidents have occurred in which corrosive chemicals have inadvertently spilled into a worker's boot causing burns to the foot.  The employer did not select and have the affected employee(s) use adequate personal protective equipment to prevent entry of corrosive chemicals into the boot.

(a)   On or about July 29, 2016, the employer did not record correctly the following workplace injury on the OSHA Form 300 or equivalent for the calendar year 2016: On or about June 24, 2016, an employee received a chemical burn; in column D the date of injury was erroneously entered as  June 10, 2016.  The injury resulted in approximately 2 weeks restricted work activity and the number of days of restricted work activity in column L was erroneously entered as zero.

29 CFR 1910.1025(l)(1)(i): Employee(s) working in an area where there is potential exposure to airborne lead at any level were not informed of the content of Appendices A and B of 29 CFR 1910.1025:  (a) On or around October 27, 2016, in the Production Area, a Production Blender transferred 2 lbs of lead acetate powder to a solution.  This worker and other workers who may perform this task were not informed of the content of Appendices A and B of the Lead standard (29 CFR 1910.1025).
