29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy. On or about July 28, 2016, the lockout and tag out procedures used during servicing and maintenance activities did not include steps for shutting down, and isolating, blocking and securing all types of energy sources, including but not limited to, electrical, hydraulic, and pneumatic energy sources.

29 CFR 1910.147(c)(4)(ii)(C): The energy control procedure did not clearly and specifically outline the steps for the removal and transfer of lockout devices or tag out devices. On or about July 28, 2016 at 1380 Morris Rd., Madison, GA: Where employees are exposed to amputation, caught by, and electrical hazards, the employer did not include procedures for the safe removal and restoration of energy to the machines or equipment in ZES Procedures for equipment including but not limited to the Hammer Mill and Chipper.
