(b) On or about 7/28/2016 - at the Combo Dump in the Marination Room, the ingoing nip point between the inspection conveyor belt and deflection/direction shield was not guarded. (c) On or about 7/28/2016 - at the Combo Dump in the Main Cutting Room, the area around the dumper was not guarded to prevent employees from entering the danger zone while the dumper was in motion.
