29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  a) On or about 7/29/2016, 8 West Main Street, Little Falls, NY:  Employees working on a roof, lowering materials on the North West side, were not protected by any fall protection equipment.  Employees exposed to falls of over 30 feet.  b) On or about 7/29/2016, 8 West Main Street, Little Falls, NY:  Employees working on a roof, assisting the owner on the South East side, were not protected by any fall protection equipment.  Employees exposed to falls of over 30 feet.       c) On or about 7/29/2016, 8 West Main Street, Little Falls, NY:  Employees working on a lower roof, between buildings, were not protected by any fall protection equipment.  Employees exposed to falls of over 20 feet.  d) On or about 7/29/2016, 8 West Main Street, Little Falls, NY:  Employees walking and working next to the top of a retaining wall were not protected by any fall protection equipment.  Employees exposed to falls of over 15 feet.     Abatement documentation is required for these items.

29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided:  a) On or about 7/29/2016, 8 West Main Street, Little Falls, NY:  Employees accessing the roof of a building from a stairwell were not provided with a ladder or other safe means of access where the elevation changed 33 inches and 30 inches.    b) On or about 7/29/2016, 8 West Main Street, Little Falls, NY:  Employees accessing and egressing the middle roof were using a fire escape system.  The employees were required to jump down and climb up an section which had a change in elevation of 44 inches with out a ladder or other safe means of access.     Abatement certification is required for these items.
