Alexander City, Alabama:  On or about July 20, 2016 and at times prior; the employer allowed his employees to work from a ladder set up on the ladder jack scaffold platform exposing his employees to a fall hazard of 12 feet to the concrete floor below.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards:  a)  At 5861 Elkahatchee Rd. Alexander City, Alabama:  On or about July 20, 2016 and at times prior; the employer exposed his employees to a fall hazard of 8 feet 11 inches to the concrete floor below in that he failed to train his employees on the hazards associated with scaffolds to include but not limited to erecting and dismantling scaffolds and the proper use of scaffolds.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:  a)  At 5861 Elkahatchee Road Alexander City, Alabama:  On or about July 20, 2016 and at times prior; the employer allowed his employees to work from an a-frame ladder which was set on a ladder jack scaffold work platform and leaned against the wall not in a fully open position, exposing the employee to a fall hazard of 12 feet to the concrete floor below.  b)  At 5861 Elkahatchee Road Alexander City, Alabama:  On or about July 20, 2016 and at times prior; the employer allowed his employees to take apart an extension ladder and use each piece for a ladder jack scaffold exposing the employee to a fall hazard of 8 feet 11 inches to the concrete floor below.

29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladder(s) were not used at an angle such that the horizontal distance from the top support to the foot of the ladder was approximately one-quarter of the working length of the ladder:   a)  At 5861 Elkahatchee Rd Alexander City, Alabama:  On or about July 20, 2016 and at time prior;  the employer allowed his employees to work from a ladder jack scaffold of which the ladder holding up the scaffold were set up at ten feet high and the base of the scaffold was set up at 5 feet 6 inches from the wall which could cause the scaffold to collapse exposing the employees to a fall hazard of 8 feet 11 inches to the concrete floor below.
