29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):  a) Jobsite: Employees are exposed to a variety of hazards, including, but not limited to, struck-by hazards. Multiple operators of the Powered Industrial Truck (PIT) on-site were not trained or evaluated to utilize the vehicle in a safe manner on or about 7/27/2016

29 CFR 1926.501(b)(14): "Wall openings." Each employee working on, at, above, or near wall openings (including those with chutes attached) where the outside bottom edge of the wall opening is 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches (1.0 m) above the walking/working surface, shall be protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system.  a) Jobsite: Wall openings exposed employees conducting framing operations to fall hazards without the use of a fall protection system.   Noted on or about 7/27/2016.
