WORKSITE: The employees, accessing the working platform of the stepladder scaffold were not provided with safe means of access onto the scaffold platform.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.    WORKSITE: The employees, painting from a stepladder scaffold located on an elevated balcony were not protected against the hazard of falling 12 feet 6 inches to the ground below by the use of a personal fall arrest or equivalent fall protection systems.

WORKSITE: The side rails of the Werner 24-foot aluminum extension ladder used to access the balcony did not extend at least three feet above the balcony railing and no handhold or equivalent was provided.
