29 CFR 1910.134(e)(1): The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace:    a)  Welding Area - An employee wearing a mandatory GVS Elipse, half face, tight fitting respirator and performing tacking and welding on a sander, exposed to chemical hazards such as but not limited to welding and chromium (VI) fumes, did not have a medical evaluation prior to use of the respirator in the workplace.    b) Paint Booth Area - Employees wearing a mandatory Sperion OptiFit, full face, tight fitting, supplied air respirators and performing priming and painting, exposed to chemical hazards including but not limited to isocyanates, did not have a medical evaluation prior to use of the respirator in the workplace.

29 CFR 1910.134(h)(1)(i): Respirators issued for the exclusive use of an employee were not cleaned and disinfected as often as necessary to be maintained in a sanitary condition:    a)  Welding Area - An employee wearing a mandatory GVS Elipse, half face, tight fitting respirator and performing tacking and welding on a sander did not clean or disinfect the respirator prior to or after use of, to ensure the respirator was maintained in a sanitary condition.     b) Paint Booth Area - An employee wearing a mandatory Sperion OptiFit, full face, tight fitting, supplied air respirator and performing priming and painting did not clean or disinfect the respirator prior to or after use of, to ensure the respirator was maintained in a sanitary condition.

29 CFR 1910.134(h)(2)(i): Respirators were not stored to protect them from damage, contamination, dust, sunlight, extreme temperatures, excessive moisture, and damaging chemicals or  were not packed or stored to prevent deformation of the facepiece and exhalation valve:  Welding Bay - Employee's respirators, which are used on a mandatory basis, were not stored to protect them from damage and contamination in that they were found on an open shelf, near welding operations and fumes.
