(a) Ontario Mechanical on or about July 25, 2016, at the address of 5066 Lincoln Hwy Crestline, OH: The employer did not ensure an operating employee had adequately secured and stabilized a structural steel load to the supporting forks of vertical mast forklift. During the travel and placement of the steel structure, employees walking/working in the immediate vicinity of this condition were exposed to struck-by hazards.

The employee was exposed to impact injuries while ground guiding a forklift transporting structural steel overhead.
