29CFR 1910.333(c)(2): Persons who were not qualified worked on electrical circuit parts or equipment that had not been de-energized under the procedures of paragraph (b) of this section. Such persons were not capable of working safely on energized circuits and were not familiar with the proper use of special precautionary techniques, personal protective equipment, insulating and shielding materials and insulated tools. Big Red Farms facility - 58535 Highway 35, Wakefield, NE - On or about July 25, 2016, an employee not trained to work on electrical equipment, attempting to reset an electrical switch for an air lock/vacuum unit in an electrical panel box that had live exposed electrical components, suffered severe hand burns from an arc flash.

29 CFR 1910.335(a)(1): Employees working in areas where there were potential electrical hazards were not provided with, and/or did not use, electrical protective equipment that was appropriate for the specific parts of the body to be protected and for the work to be performed: Big Red Farms facility - 58535 Highway 35, Wakefield, NE - On or about July 25, 2016, an employee, attempting to reset an electrical switch for an air lock/vacuum unit in an electrical panel box that had live exposed electrical components, suffered severe hand burns from an arc flash. The employee was not wearing any type of electrical personal protective equipment including, but not limited to appropriate hand, arm, and face protection.
