29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladder was used to gain access.  On or about August 2, 2016 at a commercial construction worksite located at or near 3333 Dave Ward Dr, Conway, AR 72034, employees ascended and descended to and from a roof on an extension ladder that was only extended 2ft above were exposed to fall from height hazards.
