On or about August 2, 2016 at a commercial construction worksite located at or near 3333 Dave Ward Dr, Conway, AR 72034, employees were observed performing masonry work on a fabricated frame scaffold that was not tied into the building to restrain the scaffold from tipping over.

On or about August 2, 2016 at a commercial construction worksite located at or near 3333 Dave Ward Dr, Conway, AR 72034, the employer used a side bracket installed on a work platform to support a mortar box on a scaffold that was not designed to withstand the tipping force caused by load being placed on the side bracket.
