29 CFR 1910.217(c)(1)(i): The employer did not provide and ensure the usage of point of operation guards or properly applied point of operation devices on every operation performed on mechanical power press(es):    a. Cole Tool & Die Co. dba Cole Tooling & Stamping - Ontario, Ohio: On or about July 30, 2016, the employer failed to ensure that point of operation devices located on the Seyi 550 ton mechanical power press (model HSD-550; s/n D600-014) were used during press operation.  An employee sustained an amputation injury to the hand/arm in that the horizontal presence sensing devices were not utilized when employees assisted operation by hand transferring parts within the press die.    Cole Tool & Die Co. dba Cole Tooling & Stamping was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.217(c)(3)(i)(a), which was contained in OSHA inspection number 989598, citation number 1, item number 3a and was affirmed as a final order on December 4, 2014, with respect to a workplace located at 466 State Route 314 N., Ontario, OH 44903.
