29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation: a) On or about 08/04/16 on T-2 line; authorized employees were not trained on the use of written lockout/tagout procedures when clearing dough ball jams on the T-2 line. An employee attempted to reinstall the slide chute while the conveyor was running and did not recognize the need to shut off and lockout the equipment before performing this activity. ABATEMENT CERTIFICATION REQUIRED

29 CFR 1910.333(c)(2): Persons who were not qualified worked on or near electrical circuit parts or equipment that had not been de-energized under the procedures of paragraph (b) of this section. Such persons were not capable of working safely on energized circuits and were not familiar with the proper use of special precautionary techniques, personal protective equipment, insulating and shielding materials and insulated tools. a) On or about 08/04/16 at Line T-2 ; employees were exposed to contact with electrical current (460 volts AC) when the control panel door for the index control panel near the end of Line T-2 was open and employees were going inside the cabinet to store and retrieve and a computer mouse. ABATEMENT CERTIFICATION REQUIRED
