Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of his employees employment and a place of employment which are free from recognized hazards that are causing or are likely to cause death or serious physical harm to his employees in that employees were exposed to fall hazards while working in a personnel basket on the Riblet Showcase Lift: (a) Tower #3, Riblet Showcase Lift #4, on or about August 1, 2016, and at times prior thereto, two employees were exposed to a fall hazard of approximately 25 feet to the ground when the Riblet Personnel Basket was improperly attached with a three inch spacer on the wrong side of the basket arm. (b) Tower #3, Riblet Showcase Lift #4, on or about August 1, 2016, and at times prior thereto, employees were not provided adequate training on the proper installation of the spacer bars for personnel "carriers." Note: Abatement certification and supporting documentation ARE required for this item.
