29 CFR 1926.501(b)(1): "Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a.  On or about July 25, 2016, at the site located at 2824 Brice Road, Columbus, Ohio, an employee was performing work on a billboard structure without the use of fall protection and fell approximately 27 feet.

29 CFR 1926.502(a)(2): Employers shall provide and install all fall protection systems required by this subpart for an employee, and shall comply with all other pertinent requirements of this subpart before that employee begins the work that necessitates the fall protection.  a.  At the site located at 2824 Brice Road, Columbus, Ohio, the employer did not ensure a complete fall protection system was installed before an employee began work on a billboard structure.  On or about July 25, 2016, an employee fell approximately 27 feet from the billboard structure.

29 CFR 1926.600(a)(6): All equipment covered by this subpart shall comply with the following requirements when working or being moved in the vicinity of power lines or energized transmitters, except where electrical distribution and transmission lines have been deenergized and visibly grounded at point of work or where insulating barriers, not a part of or an attachment to the equipment or machinery, have been erected to prevent physical contact with the lines:  29 CFR 1926.600(a)(6)(i): For lines rated 50 kV or below, minimum clearance between the lines and any part of the crane or load shall be 10 feet;  a.  At the site located at 2824 Brice Road, Columbus, Ohio, the employer did not ensure protections were in place to prevent the hoisted load from coming into contact with adjacent power lines.  On or about July 25, 2016, a billboard panel that was being hoisted by a JLG model G10-55A all-terrain forklift came into contact with a 7,620 Volt power line, resulting in electric shock to an employee.

29 CFR 1904.39(a)(2): Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA.  a. The employer did not report to OSHA an in-patient hospitalization within 24 hours.  On July 29, 2016, the employer reported the in-patient hospitalization which occurred on July 25, 2016.
