On or about August 2, 2016, at MAP Industries LF, Inc., the employer did not develop and utilize energy control procedures for their employees while they were required to perform maintenance by changing hose fittings and changing out the tooling and blades on the Burr Oak cut to length machine Model STC08. Employees are exposed to pinching, crushing and amputations. b. On or about August 2, 2016, at MAP Industries LF, Inc., the employer did not develop and utilize energy control procedures for their employees while they were required to perform maintenance by changing hose fittings and changing out the tooling and blades on the Burr Oak cut to length machine Model STC0. Employees are exposed to pinching, crushing and amputations. c. On or about August 2, 2016, at MAP Industries LF, Inc., the employer did not develop and utilize energy control procedures for their employees while they were required to perform maintenance by changing hose fittings and changing out the tooling and blades on the T-Drill cut to length machine Model TCC-45. Employees are exposed to pinching, crushing and amputations. d. On or about August 2, 2016, at MAP Industries LF, Inc., the employer did not develop and utilize energy control procedures for their employees while they were required to perform maintenance by changing hose fittings and changing out the tooling and blades on the T-Drill cut to length machine Model 311-36.
