29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours: a) Camp Medical Facility - An employee was hospitalized with a gastrointestinal virus on July 23, 2016. The employer failed to report this hospitalization to OSHA until July 31, 2016. b) Waterfront - An employee sustained a finger amputation injury, due to a wakeboard tow line on July 24, 2016. The employer failed to report this injury to OSHA until July 31, 2016.
