29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee use appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation: (a) BakeMark USA LLC, at 11351 E 45th Avenue, Unit A, Denver, CO: On and preceding 8/3/16, the employer did not ensure that each affected employee use appropriate eye or face protection when exposed to eye or face hazards from caustic liquids. Employees dispensed and used Delta Foremost Chemical Corporation Foremost 2001-ES Scrubmaster (containing potassium hydroxide) when cleaning the floor of the warehouse. Employees did not wear appropriate protective eye or face protection when dispensing or using the corrosive liquid. This condition exposed employees to a chemical hazard.

29 CFR 1910.1200(h)(1): Employer had not provided employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new chemical hazard the employees had not previously been trained about was introduced into their work area: (a) BakeMark USA LLC, at 11351 E 45th Avenue, Unit A, Denver, CO: On and preceding 8/3/16, the employer had not provided employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new chemical hazard the employees had not previously been trained about was introduced into their work area. Employees performed duties in the warehouse, including operation of lead acid battery powered industrial trucks, and dispensing and using floor cleaning chemicals. The employer had not provided information or training on hazardous chemicals in the work area, including but not limited to the following: 1. Lead acid batteries (containing sulfuric acid); 2. Diesel fuel; and 3. Delta Foremost Chemical Corporation Foremost 2001-ES Scrubmaster (containing potassium hydroxide).
