29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift. a) On or about August 03, 2016, and at times prior, two workers without lanyards attached between their harnesses and the anchor point of a Sky Power 66OSJ aerial lift were exposed to falling 30 feet to ground level. Note: As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated under 1926.502(e).

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b). a) On or about August 03, 2016, and at times prior, the employer did not ensure that employees were protected from falls greater than 6 feet while standing and climbing over a terrace wall, while employees were performing painting work on a residential building without any personal fall arrest systems in place.
