29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation: A) Tarpstop, LLC, located at 12000 Williams Road, Perrysburg, OH 43551: On or about July 27, 2016, the employer did not properly train employees to lockout/tagout equipment such as, but not limited to, the KMT circular cutoff saw (Model CT 350 AV, Serial # 120589002). Employees did not control hazardous electrical energy source before employees conducted activities such as, but not limited to, changing saw blades. The KMT circular cutoff saw was not isolated from electrical energy source prior to engaging in this maintenance activity, which exposed employees to amputation hazards.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): A) Tarpstop, LLC, located at 12000 Williams Road, Perrysburg, OH 43551: On or about July 27, 2016, the employer did not ensure that employees implement energy control procedures on the KMT circular cutoff saw (Model CT 350 AV, Serial # 120589002) prior to conducting activities such as, but not limited to, changing saw blades. The KMT circular cutoff saw was not isolated from the electrical energy source prior to engaging in this maintenance activity. The employees were exposed to amputation hazards. As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lock-out/tag-out device application [1910.147(d)(4)], dissipation of residual energy [1910.147(d)(5)], and verification of isolation [1910.147(d)(6)], are not implemented to protect employees from machine servicing hazards such as, but not limited to, changing saw blades.

Employees were exposed to struck-by and amputation hazards. B) Tarpstop, LLC, located at 12000 Williams Road, Perrysburg, OH 43551: On or about August 1, 2016, the employer did not ensure that one or more methods of machine guarding were provided to protect employees from moving parts such as, but not limited to, the rotating chuck of equipment such as, but not limited to, the Jet vertical mill machine (Model JMD-18PFN, Serial # 12073493). Employees were exposed to struck-by and amputation hazards. C) Tarpstop, LLC, located at 12000 Williams Road, Perrysburg, OH 43551: On or about July 27, 2016, the employer did not ensure that one or more methods of machine guarding were provided to protect operators of the KMT circular cutoff saw (Model CT 350 AV, Serial # 120589002) from flying parts such as, but not limited to, aluminum angle pieces. The employer did not ensure that parts were securely clamped prior to being cut, exposing employees to struck-by hazards.
