29 CFR 1926.651(k)(1): Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person for evidence of a situation that could have resulted in possible cave-ins, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions. Inspections were not conducted by a competent person prior to the start of work and as needed throughout the shift. Inspections were not conducted after every rainstorm or other hazard increasing occurrence, when employee exposure can be reasonably anticipated: (a) Government Contracting Services, LLC, at 2121 North Ave., Grand Junction, CO 81501: On and before August 3, 2016, the exposing employer did not ensure that inspections of excavations were conducted by a competent person daily and as needed throughout the shift. Employees were installing underground piping, with exposure to cave-in hazards, struck-by hazards from spoil piles, and lack of egress, in excavations greater than 5 feet deep. Condition exposed employees working in those trenches to cave-in, struck-by, and entrapment hazards.
