29 CFR 1926.502(d)(15): Anchorage points used for attachment of personal fall arrest equipment were not independent of any anchorage point being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee: On or about August 1, 2016, during the installation of roofing material at a job site located at 1363 E. Main St, Ravenna, Ohio 44266; the employer did not ensure employees working on a roof of a new construction GetGo gas station were protected from a fall of approximately 13.6 feet by properly utilizing fall protection systems. The employer and employee attached two 50 vertical life lines to one Temper Anchor point.
