29 CFR 1904.39(a)(2):     The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours:   On or about July 15, 2016, the employer failed to report to OSHA an in-patient hospitalization of on an employee. The employer has failed to report an in-patient hospitalization to OSHA within 24 hours.  This most recent occurrence of this took place at the facility located at Lancaster Landfill 6001 Bluff Road, Lincoln, NE where on July 15, 2016 the employer had temporary labor employees riding passenger on an ATV the host employer was using as means of hauling the dirt, rocks and debris across the bottom of the landfill when the operator accelerated while simultaneously making a sharp left turn.  The result was the ATV rocked uncontrollably, eventually flipping onto its right side pinning the right leg of the passenger underneath. The injured employee was taken via ambulance to Bryan West Hospital where they were admitted and treated that same day.  The employer late reported the incident to OSHA on July 26, 2016.
