29 CFR 1910.1030(d)(4)(iii)(B)(1)(iii): Labeled or color-coded waster containers shall be used in accordance with paragraph (g)(1)(i) of this standard. Center for Human Development, Incorporated, 289 Walnut Street, Holyoke, MA 01040: On or about 08/04/2016, the employer did not have labeled and color-coded waste containers to dispose off blood and regulated waste for an employee who cleaned up blood, and laundry containing blood, in a client's room following a medical incident.

29 CFR 1910.1030(f)(2)(i): Hepatitis B vaccination shall be made available after the employee has received the training required in paragraph (g)(2)(vii)(I) and within 10 working days of initial assignment to all employees who have occupational exposure unless the employee has previously received the complete hepatitis B vaccination series, antibody testing has revealed that the employee is immune, or the vaccine is contraindicated for medical reasons. Center for Human Development, Incorporated, 289 Walnut Street, Holyoke, MA 01040: On or about 8/4/2016, the employer did not offer Hepatitis B Vaccination to employees including, but not limited to, field operation manager, family case workers and direct care staff, who have occupational exposure to blood and other potentially infectious materials.
