29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by the use of a guardrail system, safety net system, personal fall arrest system, warning line system, or a safety monitoring system. On or about August 4, 2016 at a worksite located at or near 134 Harkrider St, Conway, AR 72032, the employer did not protect employees from fall hazards while working on a roof top that was approximately 13 feet above lower levels.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend to at least 3 feet (.9m) above the upper landing surface to which the ladder was used to gain access nor was the ladder secured at it's top to a ridgid support and a grasping device, such as a grabrail, provided to assist the employee in mounting and dismounting the ladder: On or about August 4, 2016 at a worksite located at or near 134 Harkrider St, Conway, AR 72032, the employer exposed employees to fall from height hazards while mounting and dismounting a ladder that was not extended at least 3 feet above the upper landing surface.
