29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used: South side of the greenhouses: On or about August 4, 2016, employees were exposed to an egress hazard while working on a ladder jack scaffold without a ladder for access and egress.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. South side of the greenhouses: On or about August 4, 2106, employees were exposed to a fall hazard of greater than 6 feet to the ground level below while accessing a ladder jack scaffold from a wooden plank to repair the greenhouse roof.
