29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment: (a) Jobsite, 106 Gamma Drive, Pittsburgh, PA 15238: On or about July 26, 2016, during cleaning operations on the BL3 line mixer, the employer did not ensure that the machine was turned off and shut down using the procedures established for the machine, exposing employees to amputation type injuries.
