OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by and crushing hazards while operating automotive hydraulic lifts: a) Repair Shop - Weaver automotive hydraulic lifts were being operated without an automatic mechanical device in place to hold the lift in the fully extended position at rated load capacity, thereby exposing the employees to a serious hazard, observed and reported on or about 08/04/16. b) Repair Shop - The Weaver automotive lifts were leaking hydraulic fluid and will sometimes jump or drop when they are low on fluid, thereby exposing employees to a serious hazard, observed and reported on or about 08/04/16. c) Repair Shop -The lift owner or employer allowed employees to raise cars on a lift that did not have the lift capacity marked on the lift, thereby exposing the employees to a serious hazard, observed and reported on or about 08/04/16. Abatement Certification Required

29 CFR 1910.23(a)(8)(ii): Every floor hole into which persons could accidently walk was not guarded by a floor hole cover of standard strength and construction: a) Repair Shop Bay #2 Left - Employees walked and worked near an uncovered 40" x 13" x 10 " deep lift opening thereby exposing employees to a serious hazard, observed on or about 8-04-16. b) Repair Shop Floor Center Front - Employees walked near and over a damaged drain floor grate that had two 4" x 8 1/2" by 8" deep openings in the end grate and some loose covers, thereby exposing employees to a serious hazard, on or about 8-04-16. Abatement Certification Required.
