A. Metal-Bilt Construction, LLC DBA Metal-Bilt, Inc. worksite located at 3200 McClain Rd., Lima, Ohio: On or about August 23, 2016, the employer did not ensure employees were protected from fall hazards while installing steel roof panels on the large commercial structure. Employees were exposed to a fall distance of approximately 20.00 feet to the lowest level.  The exposed employees were installing the roof panels without the use of fall protection.
