On or about July 29, 2016, at the above addressed jobsite, employee(s) performing electrical work on a mezzanine were exposed to falls, of nine (9') feet and eleven (11"), created by the presence of unprotected sides.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: a. On or about July 29, 2016, at the above addressed jobsite, the portable ladder used to access the mezzanine did not extend at least three (3') feet above the upper landing surface, thereby exposing employees to the hazards associated with falls.
