29 CFR 1910.147(f)(2)(i): When outside servicing personnel were engaged in activities in a facility, the onsite employer and the outside employer did not inform each other of their respective lockout or tagout procedures a) Snacks on 48 Inc,. Avenel, NJ- Production Floor- The employer did not inform, or obtain from the contractor hired to perform servicing and/or adjusting the cookie dough machine, the respective lockout and tagout procedures to be used when the outside contractor engaged in servicing the equipment. Thereby exposing the outside contractor to amputation hazards. Observed on 4/15/2016
